In order to complete the membership application process, please submit the following documents along with your application:

• A list of officers and key staff.
• Organization bylaws.
• 990 form or a letter from the IRS verifying your organization's nonprofit status.
• Accompanying literature that explains your organizations missions, goals and past accomplishments. (Examples include brochures, annual reports and other identity materials.)
• Organization mission statement.
